(1) Filing. Every person desiring to drive a taxicab for a company that is licensed by the City of Olathe, shall file with the city clerk an application for a driver’s certificate.

(2) Contents. The application for a taxicab driver’s certificate shall be made upon a printed form to be provided by the city clerk and shall have printed thereon such questions as will elicit from the applicant his or her name, age, residence, height, color of eyes and hair, place and date of birth, length of residence in city, county, or state, whether a citizen of the United States, whether he or she has ever been convicted of a violation of a federal or state law or a city ordinance, whether he or she has a valid driver’s license and if it has ever been suspended or revoked, whether he or she has ever been authorized by any city or state as a taxicab driver, and if so, where and whether such authority was ever revoked, and if revoked, the cause thereof, and such other information as may be deemed proper. Each application shall be accompanied by a photograph taken of the applicant within three months of the date of application.
